Чтобы задать вопрос к подлежащему,
нам не нужно ничего менять местами или добавлять.
Необходимо просто поставить вместо неизвестного подлежащего вопросительное слово who:
Somebody called Kate.
Who called Kate? – John. John called Kate.
Who called Kate? – John. John called Kate.
Правило очень простое:
если необходимо узнать, кто выполнял действие,то в вопросе вместо неизвестного подлежащего
ставится вопросительное слово who и порядок слов не меняется. (Не нужно перемещать вспомогательный глагол)
Example: Mike bought a new bike. - Who bought a new bike?

Что такое Passive Voice (пассивный (страдательный) залог в английском языке.
Для начала давайте разберемся, что такое Voice (залог) и для чего он нужен.
В английском залог показывает, кто совершает действие:
человек/предмет сам выполняет действие;
человек/предмет испытывает на себе действие другого объекта.
В соответствие с этим в английском языке выделяют два вида залога Активный залог (Active voice)— действующее лицо само совершает действия.
Например: Brasilians grow coffee. - Бразильцы выращивают кофе. (Здесь действующее лицо — бразильцы, которые совершают действие. Бразильцы активны. - Active Voice).
!!! Coffee is grown by Brasilians. - Кофе выращивается бразильцами. Кофе ничего не делает. С ним делают. Его выращивают. Кофе пассивно. - Passive Voice)
